Annual Report/Grant Renewal Application
The Annual Reporting form tracks the distribution of the commodities provided by Vitamin Angels, and must be completed in order to receive additional supplies. Once complete, a Grant Renewal Application may be submitted to apply for a grant renewal.
Recordkeeping Tools
Vitamin Angels strongly encourages partners to track the distribution of prenatal multivitamins throughout the grant cycle, which ensures accurate reporting. A Distribution Register is available for those partners that might not have a tracking system in place. This is an optional tool that many partners find helpful for internal record keeping. Please note, for privacy reasons, we will never ask to collect this register, so partners can feel free to edit and use it as it best supports their programs.
